i think the first one is more likely. People that don't have siblings wouldn't weirded out by it and the already established relation between actors probably makes it easier for coomers to relate. There are also a lot of incest anime to back that theory up.

The prevalence of this content is very new; I don't recall seeing it all that frequently even 5 years ago, and it was basically nonexistent 10 years ago. I'm sure if someone were to look up the frequency using Google trends or whatever, it would be a J curve or exponential rise. I've never watched it myself because the concept is so revolting to me that I cannot stomach it, even knowing that it's fake. With that being said, the hypotheses that come to mind as to why this has happened are:

• Average people have gained internet access in this period of time in parts of the world where cousin marrying is a common cultural practice (e.g., the Middle East, source for the doubters).
• The desire has always existed in a large enough segment of the porn-watching public as to be a market force but some law existed to prevent displaying such sexuality as a part of anti-obscenity efforts, and the law had been quietly repealed or expired.
• A few videos were made using this framing with popular or up-and-coming models. People initially watched them to see the girls, not because they cared about the incest framing, but the people/companies making misread the trend and started making more and more of it.
• It's basically the last remaining sexual taboo that (1) you cannot speak of in public, and (2) is victimless (assuming consent). Basically everything else is relatively accepted at this point, hell even "MAPs" are rolled into the alphabet soup by some people. Porn is most appealing to those who cannot get that which they desire, in real life.

But it has not always been this way, that's the weird thing. In the past the "title" applied to the young girl in porn was babysitter, student, neighbor, daughter's slutty friend, etc., there's no intuitive reason to change the labeling to be incestuous.

I agree with what some other users have written, this a fantasy that has become more prevalent in the west given that the overall number of children per family is decreasing. Not only that, but alternative family structures are also on the rise which makes it more likely for the average porn watcher to have some non-blood-related relative to which they might feel attracted. I think a part of it is that the genre casts a wide net. Normal people can ignore the fetishistic elements because it doesn't actually change anything in regards to the phyiscal aspect of the sex.
